MEDICAL CONSENT
I authorise Bay Stormers and its representatives to consent to medical treatment for me or my child when I cannot be reached to consent. I also give Bay Stormers and its representatives consent to administer the necessary emergency care to stabilise and/or improve the current injury or condition that I or my child may have sustained during activities related to Bay Stormers instruction, practices, performances, competitions or trips.
I am fully aware that any activity involving motion, height or athletic activities creates the possibility of serious injury, and I further agree to hold Bay Stormers and its staff, officers or representatives harmless for any injury or resulting expense(s). I release and discharge all rights and claims against Bay Stormers and its parties. Bay Stormers strives to provide a maximum in safety procedures and guidelines and cannot assume responsibility for any accidents, injury, illness and even death that may occur.
PHOTOGRAPH AND VIDEO CONSENT
I explicitly grant Bay Stormers and its staff, officers or representatives the absolute right and permission to use publish or sell the photographs, video, and/or other likenesses of myself or my child, in any medium, throughout the world, without any restriction whatsoever or publication or as to the copy of any printed matter accompanying the photographs so long as Bay Stormers, its staff, officers or representatives have suitable and appropriate justification to do so. Photographs will not be used of any athletes that are not suitably dressed or are deemed inappropriate. I waive any rights of compensation or ownership thereto.
Bay Stormers Cheerleader’s Code of Conduct
Accountability for one’s actions is imperative; therefore, should the Code of conduct be violated, the cheerleader will have to speak with his or her team coach and/or the head coach to rectify the situation. Should the problem continue, suspension from Bay Stormers may become necessary.
I therefore agree:
ATTENDANCE- (This does not apply for Non-competitive cheerleaders)
- To attend all training sessions throughout the FULL Season. I have been given a space on the squad and I am expected to fulfil my potential, which will be achieved whilst attending all training sessions.
- I fully commit to Bay Stormers and will not participate in any way with any other cheer squads that will conflict with my training and commitment to my squad.
- I understand my time management must allow for revision/exams/school/work and not affect my attendance to cheer.
- Should I be unable to attend training, I will give the Coaches at least 1 weeks’ notice.
- I agree to give 24 hours’ notice (where possible) if I cannot attend due to illness or injuries. Also, if possible, I will attend the lesson to watch providing my illness is not contagious.
- If I receive an injury, I will attend all classes and take part in all stretching and conditioning exercises (taking care to protect the injury).
- I understand should my attendance become an issue to the squad’s progression I may lose my space in the routine for the upcoming competition.
BEHAVIOUR-
- I will not use inappropriate language or participate in inappropriate behaviour both in and out of the gym whilst representing Bay Stormers.
- I will not train under the influence of alcohol or illegal drugs.
- I will treat all Coaches with respect and understand their decisions have the squads best interests. I will understand that Coaches decisions do not appease individuals but the squad as a whole. I agree that the Coaches decisions are final.
- I agree to always show my coaches, team members and parents the utmost respect.
- I agree that I will not attend any training session whilst in an intoxicated state and will be fully prepared for each training session. Whilst the club understands that athletes have a social life outside of practice, in order to ensure the safety of all team members, we ask that you do not drink excessive amounts of alcohol the night before training sessions.
- I will not spread gossip/rumours about the coaches, team mates or other programs whilst at the gym or on social media. I understand that if I am found to start or take part in gossip about the club, coaches or any of its members, I may be asked to leave the club.
- I agree to engage in the upmost level of sportsmanship when representing Bay Stormers at any event. Any behaviour that displays Bay Stormers in a negative way both on social media and at events, will result in immediate dismissal.
- I will not post anything on social media which will bring disrepute to the reputation of Bay Stormers, particularly when posting videos/photos whilst wearing the Bay Stormers uniform or t-shirts. This includes the use of foul language, underage drinking, provocative dancing/posing etc.
- I understand that training is not a social gathering. Whilst we want all athletes to enjoy their training sessions, the priority of all athletes should be work first, socialise second.
- I understand that if I break the commitment to the squad, I will receive a warning letter and may have to step down from competing at that competition with no refund. If I break the commitment on more than one occasion, I am aware this will affect my place on the squad for future competitions also.
- I will treat all fellow athletes with respect regardless of their skills and ability.
- I will not belittle any of my team members in anyway.
- I will try my best to make everyone in the team feel welcome and will not make fun of anyone in my team/squad or program. This also extends to members in other teams in other programs.
- I understand the need to encourage and support other cheerleaders.
- I will respect all other teams, athletes and spectators at competitions and events.
- I will NOT use social networking sites to disrespect ANYONE from the cheerleading community in any manner.
- I will not publish photos or videos of other athletes/children on social media unless I have their prior permission to do so.
- I will not wear the Bay Stormers cheerleading uniform for any purpose other than Bay Stormers events. i.e. fancy dress.
TRAINING-
- I will ensure I am wearing my Bay Stormers T-shirt(s) to every training session.
- I understand my hair will be tied back and ALL jewellery will be removed. I understand that fake nails are a hazard and therefore will not attend training or competitions with them on.
- I understand to bring plenty of WATER to training and to keep it off the gym mats. I understand that food and drink must not be consumed on the mats.
- I will ensure I am wearing white cheer trainers or indoor trainers for ALL training sessions (black for Reign).
- I understand I MUST attend training 10 mins before the session starts, so to ensure I am ready for the start time, not arriving at the start time. I also understand leaving early is NOT acceptable.
- I understand I must NOT miss the last 3 weeks before a competition- if I do so, I understand that I may be removed from the routine as it is vital we train as a squad - no refunds will be given in this instance.
FINANCIAL –
- I understand that I am committed for the full season and that all my monthly fees MUST be paid on time.
- I understand should I not attend a training session my fees will be still be due for that month to ensure the squad can be financially cost effective to run.
- I understand that lack of payments will result in dismissal from the squad.
- I understand all fees paid are non-refundable.
- I understand I will be paying for competitions, uniform and training.
- I will participate in fundraising events to help the full squad reduce their payments where possible for competitions and uniforms.
- I also understand that if I chose to leave the squad at any time all fees paid are non-refundable. This includes, but is not limited to: competition fees, uniform fees, membership fee, spectator tickets.
I UNDERSTAND THAT BAY STORMERS WILL BE TAKING ALL OF THE ABOVE VERY SERIOUSLY AND ANY OF THE RULES BROKEN WILL RESULT IN DISMISSAL FROM THE SQUAD. I ALSO UNDERSTAND THAT MY BEHAVIOUR, ATTITUDE, ATTENDANCE AND COMMITMENT WILL AFFECT MY SPACE ON THE SQUAD.I UNDERSTAND THAT SHOULD I DISRESPECT ANY OF THE ABOVE RULES MY SPACE ON THE SQUAD MAY BE PUT AT RISK.
Bay Stormers Parent/Guardian Code of Conduct
- I understand that good parent behaviour must be portrayed at all times.
- I understand that I also represent Bay Stormers at events and must behave appropriately.
- I understand that I must not interrupt classes whilst the door is closed unless I have prior consent to do so.
- I understand the coaches main job is to coach the children.
- I understand that I can contact coaches via email, Facebook or phone if I have any questions.
- I will respect and not question the coaches decisions.
- I will not spread gossip/rumours about the coaches, athletes, Bay Stormers or other programs whilst at the gym or on social media. I understand that if I am found to start or take part in gossip about the club, coaches or any of its members, I will be asked to leave the club immediately along with my child.
- I will NOT use social networking sites to disrespect ANYONE from the cheerleading community in any manner.
- I will not publish photos or videos of other athletes/children on social media unless I have their prior permission to do so.
- I understand that I MUST arrive at competitions by the times stated on the newsletter, this will ensure cheerleaders are settled and prepared for the competition.
- I understand fees must be paid on time.
I UNDERSTAND THAT IF I BREAK ANY OF THE ABOVE, MY CHILDS POSITION ON THE SQUAD MAY BE COMPROMISED.
By agreeing to the terms and conditions I accept that I have read, understood and agree to the Bay Stormers medical policy and photo/video consent as outlined above. I confirm my child agrees to abide by the rules stated in the Code of Conduct and I confirm I will abide by the rules stated in the Parent /Guardian Code of Conduct.
